Shared Living

At Enriching Lives, we believe individuals should have choices about where and with whom they live. Our Shared Living service matches an individual with a qualified host home, an individual, couple, or family, based on the person’s needs, preferences, and Person-Centered Plan.

The host home provides a safe, supportive, and welcoming environment while helping the individual:

• Build independence and daily living skills
• Participate in community activities and develop meaningful relationships
• Access transportation and nursing support when needed
• Receive personal care as part of their overall support, not as the sole service

We carefully match individuals with host homes and ensure that host families meet applicable program requirements and are prepared to provide the appropriate level of support.

Levels of Support

• Level 1: For individuals who need limited supervision, reminders, or assistance with daily activities, appointments, or medications
• Level 2: For individuals who require increased supervision, assistance with mobility, daily living activities, medications, or safety
• Level 3: For individuals who require ongoing or extensive support due to significant health, safety, mobility, or behavioral needs

The level of support is determined based on the individual’s needs and Person-Centered Plan, with the goal of promoting safety, independence, choice, and meaningful community participation.
