Supported Living

At Enriching Lives, we believe adults should have the opportunity to live as independently as possible and make choices about where and how they live, while receiving the support they need to be successful. Our Supported Living services are designed for adults who want to live in their own home or apartment while receiving individualized assistance based on their needs, preferences, and goals.

Support may include:

• Daily living support: Assistance with activities and routines that help individuals maintain their independence and successfully manage their home
• Community access: Support with getting to, from, and within the community so individuals can participate in activities, appointments, employment, and other areas of interest
• Independent living skills: Assistance with developing and maintaining skills such as household management, meal preparation, organization, and other activities that promote independence
• Personal care: Personal care assistance may be provided when needed as part of the individuals overall support plan. Personal care will not be the sole service provided
• Nursing support: Nursing Support Services may be available when identified as necessary and included in the individual’s Person-Centered Plan
• Flexible support: Services may be provided for up to 24 hours per day when authorized and supported by the individual’s Person-Centered Plan. Additional dedicated support hours may be requested when necessary
